"Home Hospice Hub" is an app I designed from personal experience with at-home palliative care. The aim of this project is to streamline communication in palliative care between for the caregivers, patients and family, thus reducing stress and improving overall care and comfort.
According to the WHO (World Health Organization), “The estimated global number of adults in need of palliative care at the end of life is over 19 million. The majority, 69%, corresponds to older adults, with most of the patients over the age of 60.”
With an ageing population and the technology available like never before, an app that can streamline communication in palliative care is well deserved.
Keeping all medications (dosages, breakthroughs, etc.), nutrition, activity and changes in prognosis up to date and accessible to everyone on the care team will help minimize missed communications and stress on the patient. Specific needs for the patient will be met, and a more productive workplace environment for the caregivers will be formed.
As it stands now, the app is designed for iPads, with the hope of it becoming available for desktop computers and phones. The patient and their family or caregivers would have access to the app via a secure sign-in system, to update information to the app. This enhanced communication will alleviate the stress and discomfort for the patient, and create a more productive workplace environment for those caregivers involved.
My first-hand experience with my own parents in this field is what inspired this project, and the desire to make the lives easier for so many others going through this is what propels me forward.
